What Is Team Training?

The Concept.
Patients with multiple conditions rely on health care professionals from a variety of disciplines. This is often the case with elderly patients. Studies show that complex patients manage better when their doctors, nurses, gerontologists, pharmacists--every health professional involved in their care (including the patient!)--work together as a team to develop a smart plan of care.

The Need.
Unfortunately, teams, let alone effective ones, don't just happen. It takes training.

GITT's Role.
GITT does the following:

  • Establishes a resource center in a participating institution. Staffing may vary.
  • Schedules team training lessons monthly (mandatory).
  • Acts as a liaison to the GITT Resource Center, located at New York University.
The experience of these model training sites has alerted health professionals to the value of team training for the care of older patients.
Fact:
Nine out of ten health care professionals fail to list the patient as a critical member of a health care planning team.
